Wellness Dental

Website Privacy Policy

Effective date: October 14, 2026

1. Who we are and what this policy covers

This policy explains how we handle information collected through mywellnessdental.com, wellnessimplants.com and its subdomains, and the forms and scheduling tools linked from them, including those hosted at go.mywellnessdentistry.com (the “Sites”), and through calls and texts that result from them.

The Sites are operated by Hoosier Dentist, LLC. Dental care is provided by Wellness Dental. Once you become a patient, your dental records are protected under our HIPAA Notice of Privacy Practices, not this policy.

2. Information we collect

Information you give us: your name, phone number, email address, preferred office and appointment times, your inquiry, and any information you choose to share about your dental concerns or payment options.

Calls and texts: call date, time, duration and number. Calls may be recorded as described in our Call Recording Notice. Texts are governed by our SMS Terms.

Collected automatically: IP address, browser and device type, pages viewed, the site or ad that referred you, and approximate location based on IP address. Optional analytics and advertising technologies run only if you turn them on (section 4).

3. How we use it

We use this information to respond to your inquiry, arrange consultations and appointments, send messages you have agreed to receive, maintain and secure our Sites, comply with law, and, only where permitted, measure our advertising.

4. Cookies, analytics and advertising

Our Sites use cookies that are necessary for them to work, such as keeping form entries together and protecting against abuse. These are always on.

We do not send your form or quiz answers, dental concerns, payment or credit information, treatment eligibility, call recordings, or scores derived from them to advertising platforms.

We honor Global Privacy Control (GPC) browser signals as an opt-out. You can change your choices at any time at Your Privacy Choices or by blocking cookies in your browser.

Google Analytics and Google Ads. If you turn on optional analytics and advertising at Your Privacy Choices, we use Google Analytics and Google Ads to understand how visitors use our Sites and to measure whether our ads lead to inquiries and appointments. These Google services use cookies, web beacons and device identifiers to collect information about your use of our Sites over time. Because our services relate to dental health, we do not use this information to build remarketing or other personalized-advertising audiences.

To learn how Google uses this information, see Google’s page on how Google uses information from sites or apps that use its services. You can view, delete or manage the activity saved to your Google Account at Google My Activity, turn off personalized ads at Google Ad Settings, and stop Google Analytics from collecting data in your browser with the Google Analytics Opt-out Browser Add-on.

5. When we share information

We share information with service providers that help us run our Sites and respond to you, including our scheduling and customer-relationship platform, phone and texting providers, and website hosting. They may use it only to provide services to us. Where a provider handles protected health information, we require a business associate agreement.

If you turn on optional advertising technologies, information about your use of the Sites is shared with those advertising providers. Under some state laws, this may be considered a “sale” or “sharing” of personal information. You can opt out at any time at Your Privacy Choices.

We do not share your mobile number or text-message consent with third parties or affiliates for their own marketing. We may also disclose information when required by law or to protect our rights and the safety of others.

6. Please don’t send health details through our forms or texts

Our forms and texts are for scheduling and general questions. Please don’t send detailed medical history, insurance or financial account details, or emergencies through them. We collect your health history securely at your visit.

7. How long we keep it

We keep inquiry and marketing records only as long as needed to respond to you, follow up as you have agreed, and meet legal and recordkeeping duties, then delete them. Patient records follow the retention rules described in the HIPAA Notice.

8. Your rights and choices

You can ask us what information we hold about you from our Sites, ask us to correct or delete it, and ask us to stop marketing calls, texts or emails. To make a request, email privacy@mywellnessdental.com or call 623-267-4097. We will verify your identity in proportion to the request; a browser opt-out or Global Privacy Control signal needs no verification.

If you live in a state with a consumer privacy law, you may have additional rights, including the right to appeal our decision on your request. We will honor those rights as the law requires and will not treat you differently for using them.

To manage Google’s use of your data directly, visit Google My Activity or Google Ad Settings (see section 4).

9. Children

Our Sites are not directed to children under 13, and we do not knowingly collect their information online. Parents and guardians can schedule care for a child by calling the office.

10. Changes and contact

We will post any changes here and update the effective date.
